2. HOW TO ENTER:

Visit Sedona MedSpa’s website located at www.SedonaMedSpa.net and complete the entry form. For purposes of the monthly drawing, "receipt" of an Internet entry occurs when the entrants name, email address, and/or telephone number is received by Sedona MedSpa resulting from the entrant clicking the "Register Now to Win" button. Automated entries (including but not limited to entries submitted using any robot, script, macro, or other automated service) are not permitted and will be disqualified. Limit one entry per person. Any attempt by any person to obtain multiple entries by using multiple/different email accounts or any other methods will void all of that entrant's entries and such entrant will be disqualified. If there is a dispute of the Internet entrant's identity, Sedona MedSpa, in its sole discretion, will determine the email account associated with the Internet entry.

3. ALTERNATE ENTRY:

No form of alternate entry will be accepted.

4. RANDOM DRAWING:

Winners will be selected in a random drawing on or about the first day of each calendar month from among all eligible entries received. Winners will be notified, at Sedona MedSpa option, by email or by telephone. Winners may be required to complete, execute, and return an Affidavit of Eligibility, a Liability Release, and where lawful, a Publicity Release within ten (10) business days of notification. Failure to return documents timely, or if prize notification is returned non-deliverable, or if winner is found not to be eligible or not in compliance with the Official Rules, such winner will be disqualified and an alternate winner will be selected.

5. ELIGIBILTY:

Drawing is open to all legal U.S. residents (including the U.S. Virgin Islands) 18 years of age and older as of October 31, 2007. Prize is not transferable to any other business or provider and will be rendered only by Sedona MedSpa. Prize has no cash value and may not be combined with any other offer or discount.

6. LIMITS OF LIABILIY:

Sedona MedSpa’s Administrators and their employees, officers, directors, affiliates, subsidiaries, distributors, wholesalers, retailers, franchise(s), independent contractors, agents, advertising and promotion agencies, successors and assigns ("Monthly Drawing Parties") are not responsible for any incorrect or inaccurate information, whether caused by website users or by any of the equipment or programming associated with or utilized in the drawing or by any technical or human error which may occur in the processing of submissions in the sweepstakes. Drawing Parties assume no responsibility for any error, omission, interruption, deletion, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to the website, or alteration of entries. Drawing Parties are not responsible for any problems or technical malfunction or any telephone network or lines, computer systems, servers or providers, computer equipment or software on account of technical problems or traffic congestion on the Internet or combination thereof, including injury or damage to participants or to any other person's computer related to or resulting from participating or downloading materials in this drawing. If, for any reason, the drawing is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Drawing Parties which corrupts or affects the administration, security, fairness, integrity or proper conduct of this sweepstakes, Sedona MedSpa reserves the right to terminate, modify or suspend this drawing at its sole option, with or without notice. Drawing Parties (i) make no warranty, guaranty, or representation of any kind concerning the prize and (ii) disclaim any express or implied warranties relating to this sweepstakes.

7. RELEASE:

By participating in this drawing, entrants agree to release and hold harmless Drawing Parties from any and all liabilities, losses of any kind, including direct, indirect, incidental, consequential or punitive damages arising, in whole or in part, directly or indirectly, from acceptance, possession, enjoyment, use and/or misuse of the prize or participation in this drawing.

8. MISCELLANEOUS CONDITIONS:

Odds of winning depend on the total number of entries and alternate entries received for this drawing. By participating, entrants agree to abide by and be bound by these Official Rules and the decisions of Sedona MedSpa, which shall be final and binding with respect to all issues relating to this drawing. By participating, Sedona MedSpa may request your name, address, phone number and email address to administer your participation in this drawing. At the time you enter this drawing, Sedona MedSpa may assumes your permission to send you future marketing information from Sedona MedSpa. If you wish to later opt-out of these emails, you can do so by checking the unsubscribe button on the email. Prizes are non-transferable or assignable, with no cash redemption or equivalent. All federal, state and local laws and regulations apply. By accepting the prize, and where permitted by law, the winner consents to Sedona MedSpa's use and publish of their name, photograph and/or likeness, address, voice, and statements made by or attributed to them, in perpetuity, in any and all media now known or hereafter developed (including, without limitation, print, broadcast and Internet), for all legitimate business purposes including advertising and promotional activities without compensation, unless prohibited by law. Should a winner make any false statement(s) in any document referenced above, the winner will be required to promptly return to Sponsor his/her prize.
