Specifically, this involves completion of the web form, in which you provide a phone number (optional), e-mail address, and name which we would use to contact you in the event that you are a drawing winner. In addition, with your e-mail address you are considered to have OPTED IN and agree to receive future email communications from Sedona MedSpa.

Use of your Information

Your name, phone number, and or email address will be used to enter you in a monthly drawing held by Sedona MedSpa, the prize for which may change according to various service or product related promotions which Sedona MedSpa may feature each month. Additionally, we will use your email address to communicate specials, new product or service offerings, and industry news which may be of interest to those who have OPTED IN by providing Sedona MedSpa with their name, email address, and/or phone number.
